$22 Fee for any lost Paperwork/Printout Requests

After the 8 April 2023, there will be a $22.00 fee for any paperwork requests this includes lost referrals or medication summaries etc, that are made outside of a consultation with your doctor. This fee will apply to all patients including those on pension and health care cards. To avoid this fee, please ensure you phone us to book a doctor’s appointment when you need something done.If you require any forms to be filled or replacement for lost paperwork, the fee will apply.
